Clinic Hours:
Clinics start at 6:00 pm on the 1st & 3rd Thursday of each month.
  • We do not make appointments.
  • Persons are seen based upon their medical need.
  • We cannot guarantee that all persons requesting service
        at any clinic will be seen.

Office Hours:
9:30 am - 3:00 pm   Monday thru Thursday
9:30 am - Noon    Friday

Registration:
Registration information and applications are available during regular office hours. Forms may be taken home and returned along with required documentation when complete. Forms and documentation will be reviewed and, if complete, appointments will be made to finalize registration.
